FRANK ZARKA:
President and Chief Executive Officer
Mr. Zarka joined Advanced Sleep Medicine Services as
President and Chief Executive Officer in July, 2010. Frank is a
team-oriented senior executive with over 30 years of experience in
the healthcare industry and an in-depth knowledge of both state and
federal healthcare compliance laws .

Prior to his arrival at ASMS, Mr. Zarka served as
President and CEO of Option One Home Medical Equipment, Inc., a Corona, CA-
based home medical equipment (HME) provider.
Mr. Zarka served as Vice President of Operations
at Praxair Healthcare Services where he had corporate staff
responsibility for a $350 million home healthcare products and
services division in the U.S.
and Canada.
At Praxair he also managed the $90 million home medical equipment,
respiratory and infusion therapy division consisting of 28 branch
facilities throughout the Eastern U.S.
with 600+ employees.

Prior to joining Praxair, Mr. Zarka served as
Chief Operating Officer at Byram Healthcare Centers and Regional Vice
President at Apria Healthcare Group.
During his tenure, Mr. Zarka spearheaded several acquisitions
and mergers while continuing to manage the P&L for multiple
divisions of those organizations.
Mr. Zarka received his Bachelor of Science, Biology degree from Santa Clara University,
located in Santa Clara,
CA. He has served as Vice President,
Treasurer and Board Member of the Washington Association of Durable
Medical Equipment Dealers and Vice President of the Washington
Association of Infusion Therapy Providers.
S.K. Mostafavi, M.D., F.C.C.P., F.A.A.S.M.: Founder
Physician, Entrepreneur, and a pioneer in the field of sleep medicine,
S.K. Mostafavi, M.D., F.C.C.P., F.A.A.S.M., is Founder and Board member of Advanced Sleep Medicine Services, Inc. Dr.
Mostafavi has been in the forefront of the field of sleep medicine as
an instructor for both major hospitals as well as prominent medical
groups in Southern California.
Dr. Mostafavi’s vast knowledge of medicine consists of being
Board Certified in multiple specialties including Sleep Medicine,
Internal Medicine, and a subspecialty Of Pulmonary Disease.

After earning his Doctorate of Medicine degree in 1980, Dr.
Mostafavi completed his internship at St. Barnabas Hospital, in New York.
Following his internship, he completed his residency training in
Internal Medicine at SUNY Downstate Affiliated Hospitals, in New York, and trained in Neurology at
UCLA’s Wadsworth
program. Pursuant to this, Dr. Mostafavi completed a Pulmonary and
Critical Care fellowship, in addition to a formal Sleep Medicine
Fellowship at UCLA and Cedars-Sinai
Medical Center. Upon completing his Board
Certification in Sleep Medicine in 1992, he became one of the first
and youngest formally trained Board Certified sleep specialists in
the state of California,
and is well recognized as a leader in the field of Sleep Medicine.

Dr. Mostafavi is a Fellow of the American
Academy of Sleep Medicine and
of the American
College of Chest
Physicians.
Dr. Mostafavi has taken part in multiple research projects and has
published articles in reputable, peer reviewed journals including Chest
and Respiration. Dr. Mostafavi is an assistant clinical professor
at the UCLA School of Medicine, and is also on the clinical faculty at Cedars-Sinai Medical Center,
where he has been placed on the outstanding teaching list. He has also
appeared on numerous notable television and radio programs, including KNX
1070, NBC, CNN, KCAL 9, and most recently on NBC’s
popular hit show, The Biggest Loser, as the “Sleep
Apnea Guru”.
Shirisha Janumpally, M.D., A.B.P.N.
Dr. Shirisha Janumpally is a board certified
neurologist with subspecialty interest in sleep medicine. She is also a
member of the American academy of sleep medicine, American academy of
neurology and American medical association. She has worked in both
academic and community settings as a neurologist and sleep medicine
specialist.

Dr.
Janumpally grew up in southern California. She went to medical
school in the Midwest and came back to southern California to
complete her training. She completed her internship at Loma Linda
University Medical center in Internal medicine. Subsequently she
completed her neurology residency and sleep medicine training at
University Of California-Irvine (UCI) Medical Center.
During her residency her research focus was on
the role of dopamine agonists (which are used to treat restless leg
syndrome) in causing impulse control behavior disorders.

Following her training she worked as a sleep
medicine attending as well as an Assistant Professor of Neurology at UCI
medical center. Her clinical interests include sleep related breathing
disorders, parasimonies, and circadian rhythm disorders to name a few.
R. Mehdian, M.D., F.C.C.P.
Dr. Mehdian is a Board Certified pulmonologist, a member of the American Academy
of Sleep Medicine and a fellow of the American College
of Chest Physicians.

Dr. Mehdian has had an active practice in Thousand
Oaks/ Newbury
Park since 2002
where he sees and manages pulmonary and sleep medicine patients at
his office. Dr. Mehdian has been active in research in the areas of
Critical Care and Sleep Medicine and his research findings have been published
in reputable publications such as ‘CHEST’ and ‘Critical Care Medicine’.
Dr. Mehdian completed his undergraduate training at UCLA majoring
in Biology and Applied Mathematics. Pursuant to this, Dr. Mehdian
attended medical school at New
York Medical
College where
he earned his Doctorate in Medicine degree.

Dr. Mehdian subsequently moved back to California to rejoin his family and
start his Internship and Residency in Internal Medicine at the
prestigious University of Southern California (USC).
Following the completion of his Residency requirements, Dr. Mehdian
pursued and completed his fellowship in Pulmonary, Critical Care, and
Sleep Medicine at USC.
Karen Cheng, M.D.
Dr. Karen Cheng is board-certified in neurology and is an active
member of the American Academy of Sleep Medicine, the American Academy of Neurology, and the
California Sleep Society. Dr.Cheng is in private practice in Laguna Hills at the Saddleback
Medical Tower
and is a medical staff member of Saddleback Memorial
Medical Center.

Dr. Cheng attended college at Stanford University,
where she graduated with honors in 1999. While at Stanford, she
studied neurobiology and dance. Subsequently, she obtained her
medical degree at the Medical College of Virginia in 2003. She
completed her internship and residency in neurology at UC Irvine
during 2003 to 2007. During her training as a neurologist, she
studied sleep disorders in patients with neuromuscular conditions.

Following her residency in neurology, she sought to subspecialize in
sleep medicine and completed a clinical fellowship in sleep medicine
from 2007-2008 at Stanford
University, which
was the site of the very first sleep disorders clinic in the country.
While at Stanford, she had the privilege of treating patients with a
variety of sleep disorders, including obstructive sleep apnea,
insomnia, narcolepsy, restless legs syndrome, and REM-sleep behavior
disorder.
Anne Rice, Ph.D., Sleep Medicine Specialist
Dr. Rice is a Diplomate of the American Board of Sleep Medicine and
a Fellow of the American
Academy of Sleep Medicine.
Upon passing her Sleep Medicine board exam in 1992, Dr. Rice served as
guest instructor for the board review course at Stanford School of
Sleep Medicine, California.

Dr. Rice has also served as board review course guest instructor,
for the American
Academy of
Sleep Medicine in 2007. Dr. Rice did her training in sleep medicine
at Sleep Center at UCLA under the supervision of Dr.
Donna Arand, a former Board Director of the American Academy
of Sleep Medicine, and Dr. Ron Harper, a former President of the Sleep
Research Society. Dr. Rice’s graduate work consisted of brain research
in relation to homeostasis and learning at UCLA’s Brain Research
Institute. Clinical work with children was in the area of behavioral
engineering at UCLA’s Psychology Department.

Dr Rice has extensive experience with neonatal ICU patients, sleep
disorders in children, lung disease patients, parasomnia and seizure
disorders, neurology patients, cardiac and heart transplant candidate
patients. She recently trained and received certification in the field
of Clinical Hypnotherapy with emphasis on insomnia.
Alan Jasper, M.D., F.C.C.P.
Dr. Alan Jasper is a
sleep specialist and board certified internist and pulmonologist. Dr.
Jasper has been a member of the American Academy of Sleep Medicine for
more than two decades and is a Fellow of the American College of Chest
Physicians and the American College of Physicians. As such, Dr. Jasper
is one of the foremost sleep specialists in Los Angeles and has been
recognized by his colleagues as one of America’s “Top Doctors”
for many years.

Dr. Jasper received his
bachelor’s degree from Yale University in 1971 and graduated from
Georgetown University School of Medicine in 1976. Dr. Jasper’s
interest in sleep medicine began during his training years from
1976-1981. His residency at Penn. State’s Hershey Medical Center
provided him with instruction in electroencephalography, while his
pulmonary research fellowship at UCLA Medical School furthered his
understanding of electromyography, both components of sleep
studies. Dr. Jasper studied several aspects of respiratory control
at UCLA and delivered the first introductory lectures on the then
“new” topic of sleep apnea in 1979. Dr. Jasper established a sleep
laboratory for a UCLA teaching program and consulted and taught
there for several years.

Dr. Jasper is in private
practice in downtown Los Angeles and is a medical staff member of St.
Vincent Medical Center and the Hospital of the Good Samaritan. Dr.
Jasper treats patients with a variety of sleep issues and in particular

Yurzul Dhanani, M.D., F.C.C.P.
Dr
Yurzul Dhanani is Board Certified in Internal Medicine, Pulmonary
(Respiratory) Medicine and Critical Care Medicine. Dr. Dhanani
specializes in the diagnosis and treatment of sleep disorders, in
particular sleep apnea, and lung diseases.

Dr Dhanani did his Internal Medicine Residency at
Helene Fuld Medical Center in Trenton N.J. and completed his Chief
Residency at the same institute in 1984. Dr. Dhanani completed his
Fellowship in Pulmonary and Critical Care at the University of
California, Irvine Medical Center in 1986.

Dr. Dhanani
belongs to the California Medical Association, the Riverside County
Medical Association and American College of Chest Physicians.
Dr. Dhanani is in private practice in Hemet, CA, specializing in Sleep
Disorders, Pulmonary and Intensive Care and
is
currently the Director of Respiratory Care at Hemet Valley Medical
Center. Dr. Dhanani is on active staff at Hemet Valley Medical Center
and on consulting staff at Menifee Valley Medical Center.
Fara Ehsan: Vice President of Finance

A true veteran, Fara has over twenty-five years of experience in
healthcare and finance, and has played an instrumental role in
building ASMS from its inception in 1994. As the Vice President of
Finance, Fara coordinates and manages a team that is responsible for
all facets of the billing department. Fara’s vast knowledge of
the medical field has contributed to creating and implementing
numerous essential symposia, in addition to being the main instrument
in acquiring insurance providership from various insurance carriers,
including Medicare and Private insurance providership.

Prior to joining ASMS, Fara had functioned as a senior financial
consultant for various successful Medical Practices, including Beverly
Hills Pain & Diagnostics, Global Medical Diagnostics, Inc., and A
& E Medical Corporation.
Fara has a Bachelor’s of Science Degree in Economics, and
earned her certification as a Programmer Analyst in Computer DBASE III
from Montreal, Canada.
She also continues to expand her knowledge, while keeping informed
with the ongoing changes in healthcare and sleep medicine, by attending
numerous seminars within the field of billing, collections, and finance.
Ray Omid: Vice President of Business Development and
Marketing
Ray Omid has over twenty years of experience in sales, marketing and
business development, twelve of which have directly been in the field
of sleep medicine. Ray joined Advanced Sleep Medicine Services, Inc. in
1996, as the Vice President of Marketing & Business Development.

Ray has been responsible for the development of the company's
sales and marketing strategies, day-to-day operations of the sales and
business development organization, public relations, contracting,
corporate communications, and general marketing activities, including
branding and market analysis.
Prior to joining ASMS, Ray was the Vice President of Sales and
Marketing for Resource Management, Inc. (RMI), a company based in Orange County, California, that was involved in
the promotion of energy efficient systems to commercial properties
and the hospitality industry.

During his tenure at RMI, Ray landed contracts with some of the
biggest names in the hospitality industry, including the Hilton
Corporation and Westin Hotels.
Ray earned his Master's of Science degree from the University of Massachusetts
Lowell, and his bachelor's of science degree in
Physics and Astrophysics from the University of London.
Ray’s passion for marketing and sales has also led him to
complete many MBA courses in these fields.
Jonathan L. Sherrill, RPSGT: General Manager and Technical
Director
Jonathan Sherrill has been actively involved in sleep medicine for
over a decade. Jonathan joined Advanced Sleep Medicine Services, Inc.
in the capacity of Sleep Lab Director in 2003, and was promoted to
General Manager and Technical Director in early 2008.

Throughout his tenure with ASMS, Inc., Jonathan has managed
numerous employees, substantially aided in the development of Durable
Medical Equipment (DME) services, including the JCAHO accreditation process,
and has assisted with company growth.
A Registered Polysomnographic Technologist (RPSGT), Jonathan was
among the first group of 5000 people to earn his RPSGT credential;
the highest designation available to a sleep technologist.

Jonathan is also a member of the Board of Registered
Polysomnographic Technologists (BRPT), American Association of Sleep
Technologists (AAST), California Sleep Society (CSS) and the Southern
California Society of Polysomnographers.
Jonathan earned his Bachelors of Arts Degree from California State
University, Fullerton.
