By using this site to place an order, you agree to the following terms of service:
If a customer selects “bill my insurance” at check out, we will contact the customer to obtain any information that is needed. We may determine that we will not bill a particular insurance payer, at our sole discretion. In that case, the customer may select to reorder, paying via credit card online and personally submitting the charge to his/her insurance company. If we do elect to bill the patient’s insurance payer, we may determine that the patient has a deductible or other amount due and we may require that the patient pay upfront via credit card prior to processing the order.
Prices listed in the online store may not represent the prices billed to insurance payers.
In the event that an item is priced incorrectly as the result or of typographical errors, miscommunication or other glitches, we reserve the right to cancel an order placed with the incorrect price and refund the customer’s money.
Right to Cancel Orders
We may cancel orders for any reason at any time and will refund the customer’s money.
